Z-TEL-H 80 is a fixed-dose combination of Telmisartan (80 mg), an angiotensin II receptor blocker (ARB), and Hydrochlorothiazide (12.5 mg), a thiazide diuretic. This combination is widely prescribed for the management of essential hypertension, especially in patients who require dual therapy to achieve optimal blood pressure control.

Telmisartan helps relax blood vessels by blocking the effects of angiotensin II, a potent vasoconstrictor. Hydrochlorothiazide assists in lowering blood pressure further by removing excess salt and water from the body through diuresis. Together, the combination is effective, well-tolerated, and beneficial in long-term hypertension management.

Indications

Z-TEL-H 80 is indicated for:

  • Essential hypertension
  • Patients not adequately controlled on Telmisartan alone
  • Hypertension requiring combination therapy for effective BP control
  • Reduction of cardiovascular risks associated with high blood pressure

Mechanism of Action

Telmisartan (80 mg)

  • Telmisartan is an angiotensin II receptor (AT1) antagonist.
  • It blocks the binding of angiotensin II to AT1 receptors on vascular smooth muscle.
  • This prevents vasoconstriction and aldosterone secretion.
  • As a result:
    • Blood vessels relax
    • Peripheral resistance decreases
    • Blood pressure reduces effectively

Hydrochlorothiazide (12.5 mg)

  • A thiazide diuretic acting on the distal convoluted tubule of the kidneys.
  • Inhibits reabsorption of sodium and chloride.
  • Promotes excretion of:
    • Sodium
    • Chloride
    • Water
    • Potassium
  • Reduces plasma volume and blood pressure.

Pharmacology

Telmisartan

Absorption:

  • Well absorbed orally.
  • Food slightly decreases bioavailability.

Distribution:

  • Large volume of distribution: ~500 L
  • Highly protein-bound: 99.5%

Metabolism:

  • Metabolized via cytochrome P450 enzymes.

Excretion:

  • Primarily excreted unchanged in urine and feces.

Hydrochlorothiazide

Absorption:

  • Oral absorption: 50–60%.

Distribution:

  • Protein binding: 67.9%

Excretion:

  • Eliminated largely unchanged through renal excretion.

Dosage & Administration

  • Recommended dose: 1 tablet once daily, preferably at the same time each day.
  • Should be taken with or without food.
  • Dose adjustments may be required in:
    • Elderly
    • Patients with mild renal issues
  • Not recommended for children under 18 years.

  • Anuria (absence of urine formation)
  • Severe renal impairment
  • Severe hypotension
  • Pancreatitis
  • Electrolyte imbalance
  • Hypersensitivity to Telmisartan, Hydrochlorothiazide, or sulfonamide derivatives

  • Take only under medical supervision.
  • Use caution in:
    • Renal disorders
    • Hepatic impairment
    • Electrolyte imbalance
    • Diabetes
    • Gout
  • Patients should not use potassium supplements unless prescribed.
  • Regular monitoring needed:

Frequently Asked Questions

A: Z-TEL-H 80 is used to treat high blood pressure (hypertension). It contains Telmisartan and Hydrochlorothiazide, which work together to control blood pressure more effectively than a single medicine.
A: Telmisartan relaxes blood vessels, while Hydrochlorothiazide helps remove excess salt and water from the body. This combination lowers blood pressure and reduces strain on the heart.
A: Take one tablet once daily at the same time each day. It may be taken with or without food. Follow your doctor’s dosage instructions.
A: No, do not stop taking the medicine without consulting your doctor. High blood pressure usually has no symptoms, and stopping treatment may cause BP to rise again.
A: Take the missed dose as soon as you remember. If it's close to the next dose, skip it. Do not double the dose.
A: Common side effects may include dizziness, fatigue, increased urination, low potassium, dry mouth, and headache. Most effects are mild, but consult your doctor if symptoms persist.
A: Avoid if you have severe kidney disease, anuria (no urine production), severe liver disease, very low blood pressure, or allergy to Telmisartan, Hydrochlorothiazide, or sulfonamides.
A: Some medicines may interact, such as potassium supplements, NSAIDs, lithium, and other diuretics. Always inform your doctor about all medicines you take.
A: No, it should not be used during pregnancy or breastfeeding. Telmisartan can harm the unborn baby, especially in later stages of pregnancy.
A: Yes. Regular monitoring of blood pressure, kidney function, and electrolyte levels (potassium & sodium) is recommended for safe and effective treatment.
