Empawish-10

Empawish-10 (empagliflozin 10 mg) is a highly selective SGLT2 inhibitor indicated for the management of type 2 diabetes mellitus. It lowers blood glucose levels by promoting urinary glucose excretion through an insulin-independent mechanism. In addition to effective glycemic control, empagliflozin is well recognized for its cardiovascular and renal protective benefits, making it a valuable choice in patients with diabetes and associated comorbidities. By reducing glucose reabsorption in the kidneys, these tablets help lower blood sugar levels and improve glycemic control. It helps lower blood sugar levels in people with diabetes when used with diet and exercise. In heart failure and CKD, it helps reduce hospitalizations and slow disease progression.  

How does Empawish-10 (empagliflozin 10 mg) work?

SGLT2 Inhibition: It inhibits SGLT2, the high-capacity transporter responsible for ~90% of renal glucose reabsorption, resulting in excretion of excess glucose in the urine.

Insulin-Independent Action: It lowers blood glucose levels independently of insulin function.

Weight Reduction & Metabolic Shift: It causes a loss of ~240-400 kcal/day in urinary glucose excretion, leading to weight reduction. It also shifts body metabolism toward fat oxidation and ketone body utilization.

Osmotic Diuresis & Natriuresis: Empagliflozin increases sodium and water excretion, reducing plasma volume (diuresis) and alleviating cardiovascular strain (reduced preload/afterload).

Cardiorenal Protection: It is thought to improve heart and kidney function by reducing intraglomerular hypertension and improving mitochondrial function in the myocardium. 

Clinical benefits 

Proven to reduce the risk of cardiovascular death and hospitalization in adults with heart failure (both reduced and preserved ejection fraction).

It slows the progression of chronic kidney disease (CKD), reducing the risk of end-stage kidney disease and renal death.

Lowers HbA1c, generally reducing it by approximately \(0.7\%\) by increasing urinary glucose excretion, independent of insulin.

Associated with a moderate weight loss (2–4 kg) and a reduction in systolic blood pressure due to its diuretic and natriuretic properties.

When used as monotherapy, the risk of low blood sugar is minimal.

Dosage & Administration

To be taken as directed by the physician.

The usual recommended dose is 10 mg once daily, taken with or without food, preferably in the morning.
